Puzzles that Require Ancient Relics

I completed riddles of the ancients, but I only have 47 relics in my inventory. Based on that, it seems I must have left three relics in a puzzle at some point and am wondering if anyone remembers where the puzzles are that use the relics. Here are the ones I know of, please add any that you know of...thanks!

The big one in Iraq.

there are 3 you get as quest rewards, did you get those?

This is really odd. I had the riddles of the ancients quest finished and gathered all relics, but suddenly Sukhothai is saying I only have 21/24, and that quest is still open. Other than the Iraq puzzle, everything else is finished. Nothing's showing on the map, but there is a ghost indicator that doesn't give much direction.

I'm guessing the relics are in a puzzle somewhere, or they respawned at their original locations.

It looks like they respawned. I found three easy ones, and I'm now on the last that was troublesome to find the first time.
