Death penalty?

Is there one? Haven't noticed if so.

So if you need to die twice to trigger it does this means there is a "cooldown" between deaths, or is it a straight counter. Eg

1st death (no penalty)

2nd death (automatic penalty, OR does it depend how long since death 1?)

I think it kicks in after 1 death, but there might be a minimum level. I think there is no penalty at all before 5, and after level 5 there should be a penalty after first death.

There is a penalty after 1 death. It may trigger after a period of time or a level. I was on day 2 at level 7 and a vulture got me at low health while I was attacking a burn victim. Personally I like the -10 to wellness better as well, but I think with A17's mechanics it would be alot worse than this new debuff is at early levels.
