Also, if they're using mods, those can give all kinds of problems if they're not written the right way. Apart from that, I don't know.
It's just the way the game is and has been for years.
Em... no, that's not the way things are. Quests are actually working quite well. If you see NO marker, either red or yellow, then you have yet to discover an entire area. The zombies are still in sleeper mode. You won't see a yellow marker which indicated the final area until you have completed each area save that last one. Until that time, once you've entered an area or the zombies have been alerted, their markers will be red. If you've not gotten a marker, then you've not explored the entire POI. You're missing an area that contains sleeper mobs. The newer POIs introduced in a20 often have more than one building and, in fact, can have entirely separate looking POIs though they belong to the same one. One that comes to mind is a small post office connected to a little hardware store or something. It looks like two separate POIs but, in reality, they are the same one.
Take your time, follow the light trail, learn the POI and you'll get the hang of it, I promise. If, however, you KNOW how to do these things, you know all the hidden areas of every POI and it's still messing up, talk to your server admin. This does NOT happen in any of our MP games.

I has this issue with multiple clear/infested quest i was flying around looking for zombies i ran the whole monument several times and still no luck but i read somewhere that buttons can be triggers for zombies spawns and not pressing these buttons would result in the quest no being completed.
However some of the buttons that are clearly meant to be pressed, even the final vault door just simply could not be pressed, (no green light on button, it was just off) now i dont know what causes these buttons to not be active.
A simple solution you can attempt during the clear of any POI is to make sure any button you passed is pressed and if you cannot press this button due to it not spawning on breaking the button will have the same outcome as pressing it. (buttons do have 7000 HP but at a higher level this takes no time at all)
I hope this can help even 1 person but if you still have issues please comment on the video linked above and we can have a discussion there and finally sort out this issue many many players seam to be having.
So look for those buttons, key hooks/cabinets, and other interactable devices if you can't clear out the last of a POI.

What you need to do is take apart the generators in the quest area, this will unlock the generator mobs.
In a night quest you would power them, but in a normal quest take them apart and the buttons and lights, doors, ect power up and the mobs spawn.
That will let you complete the quests.
If a button doesn't work take all" the generators apart first and it should after.
You can find a generator by just listening for the sound it makes to track them down.
I got to the point where I had the yellow marker but could not trigger the last attack; probably because the zombies that were supposed to burst out of the marked closet were already triggered before, while I was on the roof (the closet is on the ground floor), and were already killed. When I disassembled the poorly hidden generator two engineers indeed spawned and they were marked red on the compass, but killing them did not finish the "Clear Areas" objective and the yellow marker remained.
If I get the quest again (I don't know if it's possible) I'll try to not wander around on the roof. Maybe the sequence will finish correctly then.
