I placed a draw bridge in my base and let it down before leaving the game.
Next time, I played, the draw bridge was visually still down, but when I walked on it, I fell through and died by my own traps.
After respawning, I moved to said bridge, and changed its state by pressing action button. It then flicked a bit and went up. After interacting with it again (now visually down) it could walk on it again.
This is reproducable everytime I leave the game and come back.
For my own safety, I always trigger the bridge a few times, to make sure that the visual state matches the physics.
