Are you using 3rd party audio systems like VoiceMeter and/or EarTrumpet? If you, so need to set 7DTD to the correct output device when the game is loaded within EarTrumpet.
It is weird that you can hear the main menu music but not the in-game audio.
I guess, I will start with the obvious basic questions (and please don't be offended).
Have you changed any volume settings from their default values?
Have you checked the volume settings to be sure the in-game settings didn't get turned off somehow?
Can you still hear the music playing in the loading screen?
Are you trying to load into a solo game or a server?
Are you sure that the right bluetooth or headphones are set for the game?
These are the default settings

And its important to note that you should restart the game whenever you change these settings to see the effects. Yes, the game should tell you this for all of the changes but it doesn't.
