It's typically a sign that you don't explore enough.
Does the respawn timer only count down if you leave the region? I guess that's how loot respawn works but I turn that off. Anyway, I could spend a week in the wasteland and return to Diersville and still no zombies. For a while I'd make the rounds to every trader every restock day, and zombies were almost non-existent. I think the snow biome is where I could still find zombies and that is where I spent the least time.
Here is the spawn section for the forest biome from spawning.xml. Respawndelay is in game days.
This is in the xml data, so it can be modded. The wasteland is an exception to the rule, as with a19 the developer changed it so that zeds respawn instantly when you are there at night.
Yeah, respawndelay at night in the wasteland is 0, instant respawn. During the day it's 0.3 days.

Reverted again, re-installed the mod, and drove the exact same loop again. This time I saw 48 zombies.
Yeah, I think it's bugged and has nothing to do with not exploring enough.
