Ladders
There may be another discussion for this but how to place ladders....I dug down (straight down) and ended up in a one block wide hole, went to place a ladder on the wall and just couldn;t see it to place it....then it placed itself on the diagonal (in the corner of the square if that makes sense?) inside the stone wall and could not be used.

Do I have to use them in a larger area?

Nah, it is still there and works perfectly. Just like minecraft this game works with 'blocks' but instead they use the diamond shapes to make terrain smoother. This causes the 1x1 holes to angle themselves 'diagonally'. The ladder is on the flat face, but the shape causes it to get mostly covered by the blocks near the one you placed it on. If you want a 1x1 hole, dig a 3x3 hole and line it with cube blocks like wood frames.

Ladders work just fine this way, though you can't see the ladder so well it functions perfectly.

I agree with OP. We had issues with this tonight. Wicked issues with placement and if you mess with them at all the whole ladder is ♥♥♥♥♥♥. What we did to make it work is always build a ladder off a wood frame.Place the wood frame at the base OR top and work your way down digging enough room to place ladder and work your way down. It takes more effort(twice as much digging) but it makes a perfect mine shaft and a ladder system that won't break OR can be repaired easy.

If you are standing in the block's placement zone, you wont be able to place a ladder, or any other types of blocks for that matter.

This really should be fixed. I know people have work arounds. But IMO placing a ladder to climb out of a hole should not take any planning whatsoever. They should simply affix to any flat surface.

When you try to place a ladder in a 1 x 1 space, it will not let you place it if the player is in the way. I work around this by placing a ladder one or two blocks above me and then I use a Wood Frame to reach the ladder. I then remove the Wood Frame and while still on the ladder (and out of the way), I place the bottom rung.

Or you can just dig a space to move out of the way to be able to place the ladder.

The 'auto-rotate' function is a little wonky and if you're not facing the block just right, sometimes ladders, iron bars, etc get placed incorrectly. You can negate the auto-rotate by left-clicking and rotating the object yourself...this kind of 'locks' the placement you've chosen.

This.
You can't place a ladder in a 1x1 hole if you're standing in the 1x1 hole, your chrs. fat butt is in the way. Hard to place something on a "wall" if your face is smashed against said wall.

So you have to dig one or two more holes lower and look up to where you want to place the ladder. Or be above it. Or something to that effect. Then you can take dirt or wood frames to fill in the hole below again (jump up and place while jumping).

If you stick them to wood blocks, they work fine.
