I haven't seen such instances myself, but Mytheos mentioned in another thread that on a MP server there had been cop zombies spawn in directly on players and instantly explode.
Anything you do in the game (looting, crafting, traveling long distances between biomes), will generate heat.
Usually doing something else, like let's say gathering materials in-between looting/crafting, can and will reduce your heat map.
You can display your heatmap by activating debug mode (dm in console), and pressing F6 twice. This will give you a good idea of what exactly causes more heat.
As of 11.4 (when the spawns were reduced/changed), the heat map is a bit buggy, causing wandering hordes to spawn in your line of sight.
Hopefully it will get fixed soon. The heat map is a normal gameplay mechanic. Entities spawning ontop of you is not.
