Scream when you realize how the default game cross hair is not centered. Scream harder when you see how far off iron sights actually are.
Had the same option on my monitor so checked it myself, it is actually really amazing how far off the aim is in the game, no wonder why I miss shots I should've hit with how the Iron sights are... Yikes.
Seems for me that they cannot adjust the viewmodel properly, that would explain the terrible front sights design on most weapons too.
Hope to see this revamped one day, and that the overall front sights design is temporary. I'm loving this game, but as a fps enthusiast and a new player, this is really bothering to see this in 2021.
Some will say they are working on other things or should be, but it is a fps as far as I know, this should be a priority to work on, otherwise, this just give the feeling that they give up on this and that it will not change.
Depending on how they do their weapons and how they aim, it could be such a simple task as changing the rotation on the aiming animation and aimed shooting animation in the animation program. So if that is how they did it I just have to call them lazy as it would not take that long at all to change up all the weapons and then just compile it again.
If they do aiming by just changing the whole placement ingame of the weapons then I am not sure how they would fix it.
I do of course not know exactly how their game or engine works in that aspect so I can only assume how they could fix it.

Cheats for multiplayer
With how they monitor crosshairs work they tend to ♥♥♥♥ you over way more than help you, they are at 100% opacity, with as far as I know, no way of having less opacity so they're not that usefull.
Also not sure how that is an easy way to annoy cheaters as most programs from what I have heard does not use the crosshair or the aiming mechanic to function, but instead it just goes for the hitboxes on the opposing player.

(We need more games that do this) And sure its more about games that have no Aim marker if you dont aim
It's less about it placing the aim randomly on the screen, but when you aim dead center with the crosshair and find out that the ACTUAL center of aim is in the middle of your crosshair that is not random or intended but a flaw in it's design.
An example:
Hunt showdown has their crosshair 25% lower on the screen but still centered on the other axis, forcing players to look more up and making aiming in general a tad bit more difficult but still no problem.
As said the problem arrises when all the guns have a different point of aim which can break any game.
Has this changed since A19? In A19 I always went into the xml files and changed all the "crosshair on aim" from true to false for guns and scopes and never had trouble head shotting zombies.
Where do you find this XML? I did not know that this was a thing at all xD
