1. In Gorrister's scenario, make sure Edna drops her key before you do anything for her. You can see it spawn on the ground after talking to her for a while. If you don't get this key, you'll be locked out of a crucial room. If you don't want to make a new game or load a save, just wander into the desert to restart the scenario from scratch.
2. You can get "locked" out of Nimdok's scenario's best and most complete ending. (not a big deal as it won't affect the final chapter but if you'll miss out on maybe 2 or 3 minutes of intrigue) To avoid this, you'll need to collect a specific item from the surgery ward (it won't be there the first time you go there) and conceal it in a crate you can collect from an office. If you fail to do this, the scenario CAN still be beaten.
3. In the final scenario, Nimdok is the only one who can extend the bridge at the start of the scenario with any reliability. There are other methods of doing so but they are not well understood. Just use Nimdok to start out and load a save if he dies before you extend the bridge.
In order to take Edna off the meat hook you have to talk to her. Do it for a while ignoring the options of killing her, etc. Try to make an arragement with her and the option to take her off the hook will appear. And dont forget to take the key she'll drop on the floor.

And also you need to free the guy that at some point appears hanging on the fence and give him ether. Otherwise you won't know how to do in the end of the scenario to get the best possible ending.
There are several soft locks during the whole game, thats why its so important to save very often.
