Issues with Thanatophobia achievement
I can't seem to get this achievement. The game says you have to complete the game dying 15 times or less without using the lock, and I've done this. I'm currently on NG+6, and I've spent the past few NG+'s doing the remix battles. I've died a ton in the remix battles, but not a lot in the actual game. NG+4 and NG+5 specifically I know I completed in under 10 lives. Does the game count Remix battles as deaths or is it something else? I'd appreciate any help I can get.

You have to start a new save file and do 15 deaths or less no architect. NG+ doesn't count.

In another topic they said that you can do it in ng+, suggesting that the death counter resets.

It cannot be done in ng+1 or higher. It must be a brand new save file in the vanilla game. The mod does not award achievements.

If you're playing on PC, launch the game from the steam library and an option will pop up for the mod. It has class and balance changes as well as new zones and bosses.
