Dungeons and Degenerate Gamblers would be a funny crossover to add a fifth suit "nothings" that can't get flushes.

Changing a suit entirely as in Sonic using rings instead of diamonds, or other common card suits like swords, would be interesting; I wonder how that would have to affect jokers like Rough Gem that is clearly more diamond-themed.
For Dungeons and Degenerate Gamblers, I would add a new card edition called "glitched" and it would randomly change cards with this effect into a random card every single blind. Or maybe foil cards will always be in your first drawn hand every blind.
 
For Sonic, I would add a counter in the upper right or left corner, and every card with diamonds suit, now called "rings" will add their value to the counter. When it reaches 100 and 200, you will gain free negative eternal Mr. Bones, which looks like an iconic extra-life monitor from the Sonic series.

I mean this is actually an interesting suggestion considering the tarots in Isaac, to have an isaac flair on them. Rest of this thread seem to be just spamming any random game that comes to mind because they like them.

And to push the idea a little further: Isaac has a whole tarot deck, not just the 22 major arcanas, so all 52 cards could get an Isaac skin (the suits are hearts, coins, bombs, and keys), and iirc all cards have a full artwork (not just a suit with a number). That last part would probably be overkill though.
Only problem is that tarot cards aren't the same size than regular cards, so some tweaks would most likely be needed.

+100 to this.

I really like the idea of each deck having it's own look, beyond just the reverse-side designs. I'm much less keen on having a suite of selectable skins. To me, Balatro has always felt sort of like a "comes as it's own complete set" sort of game, like a board game. That is, you pick from what is given to you, rather than a game you personally assemble before playing, like a TCG. Because of that, it makes more sense, to me, to have a stylization system that's closer to "this is the Tarot Deck, it looks like this" than to "this is the Tarot Deck archetype, it looks like whatever you want."

It's undeniably a small thing, but I think a fixed appearance helps maintain the unique and distinct feeling that each deck has, beyond it's mechanical or compositional differences. When theming gets based around skins, and the skins can be applied to _any_ base, the only differentiating aspect then is in how it plays, and so by just that little bit, the act of selecting a deck loses some of it's importance.
