It's a real thing, rounds very rarely land on the centre of the crosshairs (usually they're somewhat close though)
In Sim mode you have parallax error, since the gunner's aiming sight is offset left or right of the main gun barrel; while in AB & RB there's no parallax because, in those modes, the sight is coaxial with the main gun.
Cannons are inherently far more accurate proportionately than smaller firearms, so this should in large be a non-factor.
The IS-2 had a dispersion of around 30cm at 1000m in each direction (vertical and horizontal) so things are exaggerated in game ( http://www.tankarchives.ca/2013/08/d-25t-artillery-tables.html )
