You can make a custom game, or participate in squadron battle. Other than that, for random matches, 4 players per squad is the limit. You can try to countdown with two squads, and hit the "To Battle" button at the same time. This works sometimes, but not enough to be viable.

Four in both cases wether you're Premium or you not, as far as i know 4 for premium was a thing way back.

No, there is no way to make it more than four. This was done for balance reasons.

I find that only 4 members in a squad is bs. You should at least be able to do 6. Not much of a number rise, but other people might want to role in a big-ish squad. And there are 15 people on a team (if my memory is coreect. I just got back after a year) and 6 wouldn't matter that much
