I think it can cause issues, because Gaijin doesn't want NA users using DMM. I know they had a special DMM giveaway where users ended up losing items from it once when Gaijin figured out who was or wasn't from JP. Quite a few threads on the reddit about it.

With that said, it seems more recently from what I can find, the biggest problem is having to pay for everything Yen, and not being able to access the Marketplace.
If something is exclusive to the DMM for Japan users, any outsiders trying to use it will get Ban/Block.
The DMM will trace you even if you use VPN to hide your IP.
This happen for FateGrandOrder JP version where they were giving out exclusive Content and people who cheat their IP claiming to be from JP gets their account Ban.
Think I recall GranBlueFantasy had the same thing.
Last time I recall some DMM related event had system asking their Citizen ID.
Japan can be stingy when it comes to Content but they are also ruthless and strict when it comes to Collaboration, IP and NDA.
