I wasn't able to make a server for a long long time, but I've found this thread a half year ago and now I'm able to make one.
1. Go to this site to see your PC's IP address.

To help you, the following names are the config file name fo the maps, cause if you wanna continue your game later, you'll have to change the last line in your config from "map base1" to anything else, like if you want to continue from Ammo Depot which filename is bunk1.bsp, you just change "map base1" to "map bunk1" and save and overwrite it.

And in the config, the difficulty was for me like set skill 0 was Easy, set skill 1 was Normal and set skill 2 was Hard, and not Normal like the guys says.
But you can check the difficulty if your bring up your status screen in-game. That's how I found out that set skill 2 was Hard.
At the set hostname line, write something after it and put it between ' " '-s.
Like me. My name is Tom and I'm playing usually with my other friend cllled Tom as well, so the first line of my config file is :
set hostname "Slaughterhouse of Toms"
set coop 1
set deatmatch 0
set skill 1
...
and so on and so on, like the topic says.

You just start your server batch-file, add your ip address to the address book and ask your friend to click on Refresh Server List, your server pops up and you're done. If it does not pop up for him, ask him to add your IP number to his address book as well and then it should be there.
And you have to use the same model as your coop partner or there will be blood and friendly fire. So watch out for that !
Before you start your Quake 2 Server by the batch file, you should change your character and ask tell your friend which model are you using and THEN start your server.
You have to be a bit tactical and not shooting at everything that moves, it will be like Left 4 Dead. But you have to be just a BIT tactical. Plus it spices up the game experience a bit.

If your friend still cannot join you, stay on your server, do not exti the game and ask him to bring down the in-game console which can be done with the ' 0 ' or ' ~ ' and ask him to type "connect ip INSERT YOUR IP ADDRESS HERE " and press the Enter button.

Start by finding the manufacturer and the name of your router (just pick your router and search for these data on the bottom of the router), then select Quake 2 from the list on the right and choose your router manufacturer and the exact type of the device on this site and it will show exactly what you have to do to be able to run Quake 2 in co-op :

Then try to ask your friend(s) to launch Quake 2, let them enter their game (they start NEW GAME or LOAD GAME and enter the game), and tell them to connect to you by your IP address.
I hope this helps. I was able to play after using this page and selecting my router manufacturer and type correctly.
PS : I suppose if you've already searched for Quake 2 coop and you are on these forums, then you've already searched for it and found it, but this page is helping you tremendously to set up your server.

"quake2 +connect IP.OF.SOMEONE.NUMBERS +exec co_op.cfg" for automatic start in the first map of the game , with the enemies , specifying the skill ???

The "map" line can be changed to any valid map name if you want to start on a differrent map.
There is no need for the list of maps at the bottom of the file.
"+connect IP.OF.SOMEONE.NUMBERS" and "+exec co_op.cfg" are client/server options such that you use "+connect IP address" to connect to a different server and "+exec filename.cfg" is used to specify server options and start a new server with those options.
This is my servercoop.cfg file which i place in BASEQ2 folder:
I use the source port R1Q2 to host the game because it allows me to save and load multiplayer coop games. I'm not sure if the other ports support that.
I know this thread is ancient. But how do you save co-op progress on a dedicated server with R1Q2?

To make it a little bit easier and clear to find out is there is some issues between connection - RadminVPN (separate app from Radmin) or GameRanger

Decptie GR is very old and good, RadminVPN is better (less tech issues, more easy)
But both do emulate LAN network. You need internal GameRanger or Radmin IP adress put in the game.
Game Ranger exist from time Q2 released. Mplay HeatPlay WON and many else are predecessor's for GameSpy. Xfire appear at same time also be mentioned by debs but never be commercialised to debs as official servers hoster.
Anyway. Some of them maybe work for Q2 back in '98. Game itself have only lan, direct-ip and some servers finder without master-server in terms nowadays master-server mean.
You NO NEED anything to play coop with friend. Game Ranger or RadminVPN are optional tools check quality of connection before game itself and making private rooms. During game you put friend IP to connect. Internal IP or such radmin IP. In any case hoster is a person with dedicated port opened connection. No mobile network from smartphone. And no 3rd party solution allow to play if both have limited internet connection (public WI-Fi another example).
Just put IP address. Q2 support TCP-IP direct connection (IPX are unsupported by nowadays Windows but TCP-IP supported and there is no IPX in Q2 (only in Q1)).
