In my opinion level is not a big issue with ultimate. Ultimate difficulty is a gear check. if your gear sucks, you will suck in ultimate.
farming route(go for bosses + factions/possible nemesis spawns): Salazar(secret hideout near burrwitch outskirts), warden, arkovian undercity, cronley's hideout, bee den next to twin falls portal, dermapteran queen(rotting croplands);
optional - conflagration, blood grove(Bolvar), Zaria(darkvale village), commander lucius, loghorrean
in AoM: Gloomwald, Ugdenbog, Steelcap District
Idk about FG, haven't played it much yet.
I'd go for the basegame stuff and If you're really unlucky eventually you'll get possible Nemeses which increase your chance of cool drops.
Also check your faction vendors as it is, you may have unlocked some stuff you could make use of, like for example fix your resistances.
Level for ultimate doesn't matter much, just try it out and if can kill things without dying much, then you're ok. Otherwise you might stay on elite until 90+ especially if you're on hardcore. At least that's some of what I've seen from people playing hardcore. After a while you might start noticing the same/similar items dropping on elite and blues/purples won't drop on every run. If you can, do AoM on elite. You'll get nice green items that can help on ultimate.
Farming in any of the dungeons is ok too, if you have the materials and don't die.
Sth I want to point out, when you get past Cronley's hideout on ultimate(maybe it's when you kill Cronley) Moneybags will stop dropping dynamite on elite, so you'll have to get it on ultimate, if you want to farm him for it. Idk if it's intentional, seems like it.
Once you have the resistance gear, going to Ultimate is highly recommended though. Loot and Exp are much better there.
