The wooden thing, with all the other "things", transports you into a brand new, randomly generated world
Caves are kinda like, it's almost like an entirely new world, because they are usually absolutely frickin huge. There are different plants, different animals... mainly hostile animals, but nonetheless. Personally, I never much cared for cave exploration, but if you've not really done it I'd say go for it.

I've not really played about with the woodern thing much in sandbox, but as far as I'm aware it just makes you a new world.
Maxwells door will take you into what you could call story mode yeah. You should try out adventure mode, I think it's quite fun. :)
