Giant Toothbrush in Panic Room?

So I've been trying to look for this trophy and I still can't find it. Is it locked in a specific Difficulty? or What floor does it usually spawn?

Check bathrooms primarily, but I've seen it in corridors as well. I'm not sure if it's a 100% chance but I'm pretty sure the spawn chances are decent, at least 50%.

If you can't find it, try restarting and hope you'll have an easier time. Alternatively bring a group to search with you so you can find it faster.

(Also, I THINK it's locked to Overkill and above? Not sure.)

Edit: Oh, and as for difficulty, it's on all of them, even Normal.

I found it every single time so far. the thing is , it doesn't have to spawn inside ! check the alleys around the building. i found it twice next to a garbage can. Most spawn points are inside , but if you can't find it anywere , search outside (or just do that before going in the first time).
