Full speed swarm increases difficulty even on lowest settings?
I get 20fps in game with FSS on lowest settings, and cops are more crazier than on my friend's 40fps game..

FSS work the best at unlimited fps setting or on your best fps setting.
So if your pc run the best at 20 fps, setting the FSS at 20 fps or unlimited would make the AI react better than having it at lower or higher fps.

Don't forget that adjusting your frame rate limit at video option seems to effect it as well.

At least this is all from my personal experience after using & tinkering with FSS.

From what I heard about the mod, it kinda does. The Full Speed Swarm Mod make sure the enemy A.I. performs well regardless of the player's average frame per second. Unfortunately, I can't explain the mod in detail because I don't normally use it(and I'm going to be away from this game for a while). However, there is a user that did explain it very well and that same user made a Google document about it. TdlQ's goal for FSS was to unliknk cop reaction times from the host's FPS, so it should in theory work the same regardless of the chosen framerate cap.

Yes, though it's actualy the other way around. The Vanilla game's difficulty is HEAVILY reduced by low or even normal framerates, with cops often reacting far slower than Overkill's own settings dictate they should. With FSS, cops will always react as fast as intended, which makes the game considerably more difficult than Vanilla.

FSS also fixes a bunch of spawn bugs causing cops to stay outside the map and block spawn points permanently while still counting towards the spawn limit. The result is more cops in circulation who are also better at finding you and better at reacting to you.
