Floating objects/items

I've already seen a revolver floating mid air and yesterday a floating motion tracker, it disappeared as soon as i took out my motion tracker.

Anyone been seeing stuff like this too?

Yes, I get this if I reload an area multiple times, I kept going back to all the other areas trying to unlock doors every time I got an upgrade to the torch or hack tool.
