To install it simply unzip it, open the folder, right-click and copy the DATA folder, then open up your Alien: Isolation folder (default c/drive>program files x86>steam/steamapps>common>alien isolation) and paste it in there. When it asks if you want to overwrite, just say yes or yes to all.
You need to start a new game for it to begin working, it won't work if you load your game and try to continue with the mod now installed. Here's what the mod does exactly (I've played it extensively):
It takes the tether off the alien. The alien is no longer tethered to Amanda and is now free to roam the entire map that you're on. If you make a loud noise, like firing a gun, the alien will still come running, but you can actually run now if the alien is far enough away. This will make the game quite a lot easier but, I find, also makes it more tense. It's scarier when you don't see the alien for 3 minutes and start to worry about just where the hell it is.
You may not need to start a new game to get it working if you don't feel like going through the earlier levels again. You could try starting the mission again from the beginning - instead of loading/continuing your game from the main menu, choose to select a mission and start from the beginning. This may work. You'll know it's working if you crank the difficulty up to hard (can be done from options menu mid-game!), find a place to hide, turn up the volume and listen. On hard, without the mod, the alien should be tethered very close to you at all times where he is present. You should be able to hear him moving at all times, either down on the ground or up in the vents.
If the mod is working, listen closely and see if the alien's footsteps/movements sound very distant at times, or if you can't hear them at all for 30 seconds or more. This is easier to do if you turn off the music (and turning off the music also makes the game more immersive in my opinion as you can hear the alien + the ambient sounds of the ship more clearly). If you don't hear him then it means he's all the way on the other side of the map looking for you.
If not then yeah, I'm afraid you'll have to start an entirely new game to get it to work.
Sorry, I've never used a Mac. I've never even physically been in the same room as one. The way to see the folder on Windows is to right-click the game in your STEAM library, and from one of those boxes you can click to open the game's main folder.
Edit - right-click>manage>browse local files. That will open the folder. Or... like I say, that's how it works on Windows. Max is alien to me (no pun intended).
