Ghost achievement broken

I've completed every mission in consecutive order and got the ghost and didn't kill tick for every single mission but the game still won't give me the ghost achievement is there any way to fix this without replaying the entire game again.

Ghost and Clean Hands should be combined together because the Ghost achievement description is confusing. You are guaranteed to get it doing a full playthrough of no kills and no detections.

Its not broken, its likely something spotted you which you didnt realise. You can install a simple mod which allows you to see in real time if you have been detected or not which helped me massively. The only broken achievement is clean hands/shadow which bugs during the granny rags finale

Every single mission is ticked I went back and checked and I ran that mod for my entire run and it never gave me a killed someone or spotted notification

The in-between missions at the pub count too, and you must do it both in a single, unbroken run.

Mission Select is NOT going to trigger Ghost or Pacifist.

Seen this trip up a lot of folks over the years, so you're not alone, if that helps.

I did do it without using mission select what can you do that will mess up clean hands/ghost at the inbetween areas

It may have been during the aforementioned granny rags finale, that part bugs for people like it did me

There are Weepers in the sewers in between missions that can detect you.

You cannot be seen by any hostile enemy. Weepers count.

If you didn't get it, then you didn't do it right

I didn't get it due to some kind of bug or issue with the weepers after mission 2, I didn't get seen or kill during the missions but either granny rags or the hound pits weepers messed up the achievement

I didn't get it due to some kind of bug or issue with the weepers after mission 2, I didn't get seen or kill during the missions but either granny rags or the hound pits weepers messed up the achievement

More than likely the weepers ruined it for you. Interacting with Granny Rags outside of her fight will not ruin the achievement for you.

It's not broken, probably someone spotted you (as others already stated). I got clean hands, ghost, shadow, poetic justice, mostly flesh and steel, specter, faceless, surgical and vanished all in the same run. Plus a few others that I could have done in any other run (like reaching the Dunwall's bridge top). It's been quite hard, and even a bit funny doing it.
All I can suggest, is to keep doing saves. Every enemy you decide to takedown silently, after placing it in a safe position (if rats eats it then it'll count as a kill done by you) you should save. Same goes after reaching a new safe place, just keep saving (and do more than a single save) so if someone sees you, you can just reload and do a better job on goin undetected.
A single save could do the job, but I got caught without noticing it and saved again. I was on the third mission, but luckily I had a previous save (like 15 minutes before), reloaded it and after completing the level it gave me the "undetected" mark.

If you're talking about in the sewers, you can throw the body to the rats. The body is already dead, you didn't do anything.

Chances are OP was careless and was maybe either spotted by weepers and didn't realize or had an unconscious body eaten by rats and didn't notice.

For some reason the dead thrown body gets counted at the end of game, really weird

but as you said , he didn't care about weepers

Maybe the game sees the beginning sewers as a requirement where as at the end a conscious choice?

No clue, when I was doing my Ghost / Clean Hands / Mostly Steel run, I had abandoned my sword at the time and was purely relying on Blink so I probably just skipped over the section you're talking about.
