Mission Select is NOT going to trigger Ghost or Pacifist.
Seen this trip up a lot of folks over the years, so you're not alone, if that helps.
You cannot be seen by any hostile enemy. Weepers count.
If you didn't get it, then you didn't do it right
As The Pug Hybrid said, do this achievement with Clean Hands, kill absolutely nobody and this achievement is easy.
All I can suggest, is to keep doing saves. Every enemy you decide to takedown silently, after placing it in a safe position (if rats eats it then it'll count as a kill done by you) you should save. Same goes after reaching a new safe place, just keep saving (and do more than a single save) so if someone sees you, you can just reload and do a better job on goin undetected.
A single save could do the job, but I got caught without noticing it and saved again. I was on the third mission, but luckily I had a previous save (like 15 minutes before), reloaded it and after completing the level it gave me the "undetected" mark.
Chances are OP was careless and was maybe either spotted by weepers and didn't realize or had an unconscious body eaten by rats and didn't notice.
but as you said , he didn't care about weepers
No clue, when I was doing my Ghost / Clean Hands / Mostly Steel run, I had abandoned my sword at the time and was purely relying on Blink so I probably just skipped over the section you're talking about.
