The hit percentage isn't necessarily accurate on how it works. Generally a 50-60% shot will be a miss.
Some basic advice:
Go slow!
Never take a shot under 80% and even then, always have a backup plan. Assume a soldier will miss, what will do you after he misses to either elminate the threat or either smoke or move the soldier into cover (obviously most shots end that soldiers turn so you have to take that into effect.)
Try to flank enemies.
Never run.
Never go into fog of war areas with your last soldier in a turn.
Those are some basics.
If you have the time and want to watch some impressive Let's Plays. Watch Zemalf (either his second Enemy Unknown Impossible Ironman run or his Enemy Within Ironman run. Both are amazind and his tactics and slow play (while annoying at times) show how to dominate this game. Smart play makes this game easy.
Also, what was your hit %? Anything less than 100% has a chance to miss obviously.
I will say that missing at point blank is very hard to do since you get a pretty decent aim bonus with assault rifles and a massive aim bonus with shotguns.
I have countless numbers of times where a shot gun missed at 100% and at point blank range. At the end of the day it still comes down to the RNG, which is poorly done.
The others have already given you some tips but let me tell you the logic of why you missed when you were right next to the enemy.
Everyone is always moving,despite what it looks like everyone is in the heat of battle,you just give orders at critical moments,that enemy that was right next to you? he was in the process of moving away and shooting that soldier,that's why it's easier to shoot enemys next to you but does not garuntee a 100% chance to hit.
The reason you can miss at point-blank range is that shooting is abstracted. Many relevant factors are not considered individually, and are simply lumped into your hit percentage. There are lots of reasons "in reality" why you might miss a shot even when you are standing right next to someone.
In XCOM there is only one reason why you miss: because your hit % was not 100.
Stop lying, 100% shots do not and cannot miss.
End of the day its RNG...a dice roll.
I normally throw grenades not dice. Who needs RNG when you can just blow up the whole map.

One of my BCS instructors told our class a story - During the Vietnam War, his unit was on patrol. HIs point man is moving forward aways ahead of the squad and turns around the corner of a building. The squad hears a shout and a ♥♥♥♥-load of gunfire and a second later the point comes running back around the corner.
He had come around the corner and was F2F with another enemy scout and they unloaded their rifles at each other. Swore up and down that he had killed the guy. The squad pops around the corner and there's 5.56 and 7.62 brass everywhere and not even a bloodstain.
Now imagine that scenario but the NV trooper is replaced by some snarling alien monstrosity.
