Can someone kindly explain to me private servers?

So I've been seeing all over that people host/own private servers, where they can freely spawn in enemies, gear, custom gear, but what really caught my eye was that they were able to play *alone*.
It's nothing I'm ashamed about, but I am a big, BIGGG wuss - even though i'm a knight with maxed def I'm still scared as hell to go into mad labs or any dungeon harder than an abyss. I thought this would be a great way to test out dungeons, practice, and try to get better without having to risk the pain of killing my knight which I have worked so hard to get (I see people consider deaths in the game to be just something you have to go through once in a while, like I have guild members who max their characters and then die 2 weeks later and just start doing it again - I however don't like to rush getting gear, I try to take it slow so I don't get into trouble in the heat of the moment and die or anything of the sorts).

I'd really just like to know the general details, such as:
Do these servers cost money to host? Even if only you play on them?
Can you even get private servers if you are a general player like me, or do you need to have something special of any sorts?
How do you get custom gear, do you need to make it yourself or can you download it for the server like it's a mod for the game or something?

This isn't the place to discuss that. Steam has a very strict anti-piracy policy. Stealing (even if it's just a game) is still illegal.
