Basically, all you need to do is get to the end of the game, around the Empyrean, and do all remaining quest lines, similar to how you unlock the True Neutral ending in Canon of Creation. Specifically, you need to have cleared the first Mastema quest by beating Belial. Afterwards, go back to where Mastema was and you should have a new quest, which will give you a boss fight against Satan. Beat this boss and you will unlock the Miracle that raises the level cap to 150. This is independent of Godborn mode, however beating Satan will also unlock Godborn mode if you then go about beating Lucifer as normal and select NG+.
You *do not* need to be on NG+ or clear CoC first, yes. But basically you just need to have a completed save file anywhere. So you can go through the game, save before you beat the final boss, beat the final boss, save in a second slot, reload the first save, and the Mastema quest and subsequently Satan boss will be available. Beat him to unlock Godborn option for NG+.
