V-sync is absolutely required or physics will completely flip out, scripts will break (like the door) etc. But I am playing the game at 200 FPS with V-Sync without issues (other than very minor physics bugs here and there).
Easy to enable with NVidia, but not sure about AMD users
If AMD, I can't help you there but I remember reading/hearing that it doesn't really work with AMD so you might be out of luck if you do have an AMD.
Then I am doubly lucky. I have Nvidia, and I got it working by changing the settings in Control Panel, instead of (only) in the game. It is working, I have my first save, and I can now experiment with different settings.
