Question about Giangio (SPOILERS)
Was he faking his illness? Also, who is he really, just somebody watching over the events happening in Krat?

Yes, he seems to be an agent of some organization which is chasing ways to become immortal, or ways to transcend humanity. His real name is Pareselsus, his job in Krat seemed to be to observe P coming to life and collect golden fruit for study.

It feels the devs intentionally left that for interpretation in order to expand in the DLC or in next games who knows

Given that he created the weapon, the Uroboros' Eye and judging from the description, "Giangio" isn't some sort of meekly, low-time alchemist either, but arguably one of the most revered.
