Ouruboros' Ring maxed and 2x acid grindstones with the perk to last longer, Viktor's amulet plus one for damage boost per Fable Slot, and the best armor I could get by then for the most part - using Puppet String lvl3. Also had the perk to get fable on healing, and would assure I started the fight with max fable (5 bars).
Strat was - walk in, immediately use first grindstone, then dodge if needed or spin up the ring with it's second fable art; Puppet String link attack if he is too far, else I would just hit him between his combos and would dodge through the attacks to get almost behind him (but not all the way to try and eek out the 3 hit combos). Usually needed to use 2 heals as I do trade on some of the hits where I would get some extra value (like procing acid or about to stagger/phase change). Generally took between 2-4 minutes for this phase.
Phase 2 was more of the same, but I would lean a bit more on link attacks with puppet string when he used certain attacks. The leap into the air is protected once you get high enough, and so you can effectively vault over sweeps and sometimes the magic (not hand obviously). Use second grindstone near the middle of the fight and keep that blade spinning as much as you can as it does 3 hits per connection (stack that acid). Ended up taking around 8-10 minutes for this phase as I also back-peddle all the way whenever he uses that hand spell.
Then - if he was close to stagger phase 2 - toss the blade at him with the first fable art, then try link attacking and he would be close to staggering for some extra damage. Toss stuff as needed.
My P-Organ was tuned for anything doing more stagger damage/increase stagger window also, so that may be why I had somewhat of an easy time staggering with the Puppet String / spinning blade bit.

I'd hit him once, if I see he put the mace back to his shoulder, i'd do a 2nd hit
i know doing 1-2 hit at a time will make the battle so much longer,
but that's how I did it, gotta keep them brain focused
