yes, there are no downsides. it seems like there could be but no, you won't fail any other sidequests or get kicked out of any other alliances. it's necessary to progress many sidequests and a lot of good armor, a merchant, and other items are gated behind it too.

you can't fail any other major quests by progressing trough the side ones. this is a side one.

dude trust me. i'm pretty much a seasoned souls vet (been playing ER since patch 1.6 way back in '23). i'm here to tell you there is literally no downside to doing volcano manor

If you're ever given a choice in a modern FromSoft game, ignore the text in the choices and read the top choice as, "Start / continue quest," and the bottom choice as, "Ignore quest." There are no ramifications at this stage.
