Being maidenless refers to being without a Finger Maiden, who serve an important role in the Lands Between.
Varre does not use it as an insult (not straight away, at least), but as a statement to describe the state the player character is in, to give the player a clue about how the world they play in works and what to do next.

To answer your question: yes, he will refer to the player as maidenless independent of their sex.

Also note how the whole thing can be interpreted as an ongoing meta-commentary (or joke) about the usual concept of Souls games having a maiden of some sort who levels you up, including Melina's comment about how she is not a proper maiden but can serve in that function if you agree to her terms. In the ER world the Tarnished used to be delivered with their level-up persons, but yours was murdered, so Varre mocks you and you have to improvise.

it's implied that "spirit girl" wasn't tarnished. the item description for her hood states the she never saw the Guidance, so there's every possibility she was a normal human that basically got exiled for political reasons, and claiming that she's "tarnished" would be a convenient excuse

But I mean that could very well mean that she is Tarnished, but just unable to see the guidance of grace, like many others.
From Brother Corhyns dialogue:
By the way, do you still see it? The guidance of grace.
You do! Wonderful news. Most Tarnished are blind to it these days.
You are something of a rare breed.

Also directly from something that she tells the player herself:
I never knew the guidance bestowed upon us Tarnished had such fantastic roots.
Although, it's all a bit much for me, in truth. I'm still looking for my own purpose.

Yeah, I tend to agree although I imagine that maidens were never numerous enough to give every Tarnished one even in the good old days, which is why a special note of your character being of no renowned is made in the intro (so what made you special enough to deserve a maiden?), but I'm curious how most of the Tarnished in the Hold got thier invites.
