It's normal. The tear only reduce the heal drain from his nihil.

Yeah he healed on me, but the amount was a lot less when using the the physics tear. Didn't realize at first I was to use it but caught on real quick after remembering that I had the tear in my inventory.

Yes, that will happen no matter what unless you can kill him before the incantation goes off which isn't easy unless you've the build for it. The Purifying Tear only protects YOU from damage. His healing comes from the form change he gets from the Mother of Truth.

It's a hard boss (ofc not for summoners). If you learn his move set it doesn't matter if he heals. If he start to heal you should counter it with bleed or a hard hitting weapon art.

So, apparently vow of the indominable (ash of war) is another way to 100% evade his nihil attacks. Though I don't know if he still heals, considering the player doesn't have a hitbox during the animation.
