There was no warning or logic to it at all.
Well, seeing as this is how the quest goes if you do everything correctly I can understand that that happened :))). The quest continues with her father .
That said, Irina's dad is up his tower or he is at the spot where Irina is, haven't had him break on me in 5 playthroughs now. I did the quest after killing the end boss in my first playthrough where I also had already killed the boss of Castle Morne, and even then nothing broke on me with the quest - required quite a few reloads of the area this way, though.
Personally even if that blanked out part is true, and pretty sure it is, that in no way completed the quest AFTER going back, and speaking to the father. He just repeated the same 'i'm hopeless' bs over, and over till I got sick of it, and killed him.
I did try leaving, and returning on several occasions prior to that though, so that's null n void less a bug I think. Either way it's a bs broken quest imp
Again, it's not broken at all. You're apparently just not really capable of wrapping your head around the fact that the outcome is what it is when you do things correctly lmao.
I just did this too. You can talk to him, even after beating the keeps boss. He is sitting on a bench on one of the tower like areas. You just need to talk to him, exit to menu, go back and talk again, then he will say he is going to find his daughter.
The re-load is because he will just repeat the same thing over and over, waiting for the trigger for the next part of the quest.

At least I do not think it's a bug. Just a byproduct of the somewhat clunky quest design and you doing the single quest steps 'out of the intended order'.
That said, most if not all quests do have some kind of point of no return where, when you kill boss A or change the world by killing boss B, the quests are no longer available. That's just how quests in these games work.
