Or possibly a difference could be that you switched on Steam input and forgot, which is now for some or other reason misbehaving?
Very slight chance that it's an FPS-issue and that e.g. you disabled VSync so that it's now too high?
But, okay; there's an abolute ton of FPS-issue (see e.g. https://github.com/GTAmodding/GTAIV-Issues-List/issues/112) so it sort of needs doing away with first, but I agree that your issue likely isn't.
Still. FusionFix fixes (a lot) more than FPS even, so it's also not really useful to troubleshoot anything without looking at it first. I.e., I advise installing it as per that linked issue and seeing if it helps any. Uninstalling (which you shouldn't do, but...) is just a matter of deleting again what you copied in.
