to fix your game performance use my ini.
I use 1440p. so if u have another resolution u need to change it manually.
Just copy it in your Fallout76 Documents folder.
i also recommend using Rivatuner to cap ur fps to 60.
with this ini and Rivatuner i have 99% 60fps :)

I'm a get a solid 120fps on my 1440 monitor on ultra/max
It is for Fallout. When 144hz is enabled im getting 48fps for some reason. Disabled v-sync and now i can have 100+ fps, but locked for 60.
I have GSYNC running on my 165hz panel
Most screens have frequencies lower than their max frequency that are ideal for people whose graphics hardware can't output at the maximum frequency, for a 60Hz monitor there is 30Hz (and occasionally 45Hz) and on a 144Hz monitor they are 48Hz, 72Hz and 96Hz.
These timings are just divisions of the max frequency by 2, 3 or 4 that the screen driver (a chip inside your monitor) is comfortable running at, and so tearing is less of an issue.
It used to be that you had to cap it yourself by setting the game or application to 30fps if your GPU couldn't output at 60fps but these days G-Sync and Freesync do it for you by detecting the average frame rate of your GPU and lowering it to one your screen prefers. In your case your GPU couldn't manage 72Hz so it dropped it to 48Hz.

It's doing that because you probably can't sustain a consistent 72 fps (most people can't with higher settings no matter what your hardware is)... so it would be choppy, stutter and be horrible as frames jump around, without the lock.
You will have a better, smoother experience if you stick with 60hz.
My initial problem was that when screen refresh rate is 144hz game runs in weird 48fps. And I also had some mouse lag.
Proper solution is disabling v-sync in INI file. Now in have 100+ fps. But! Since I don t need it, I locked max fps via msi afterburner for 60 fps. Now it’s perfect. And tearing is solved via g-sync.
