If you have a high end pc you can go ultra size, but keep in mind it is going to change the balance of the game.
If i remember correctly ( correct me if im wrong ) if you play on ultra BIG single entity will have a hard time since there are more unit hitting them and spells will indirectly be stronger.
Wh3 seem different in the scaling but i need to test ultra size more before being 100% on that.
I mostly play Large since it is what i was using in wh2.
For the fun factor you should probably use Ultra if you have a good pc.
Then it also depends a lot on your PC config if it can support it or not.
Run ultra if you can or large if you want the performance headroom.
While Total War: Warhammer 2 was balanced around Large unit sizes.

For single entities it is different. On Ultra a lord will have more health and deal more damage(weapon strength) compared to large, medium or small(each having their own values). Same with single monsters etc..
Damage spells and abilities also scale depending on your chosen scaling.
