The way to counter it is to declare war on the major Tzeentch faction.
He can't transfer your settlements to any faction you are at war with (they seem very unlikely to do it if they can't transfer the settlement to themselves).
Thanks but I haven't discovered the major one yet ;( Ugh and heroes indisposed too atm gurr. It is ridiculous they can do this to someone they haven't met yet. Thanks for the solution tho.
I edited my original post. Only the major Tzeentch faction should be able to do it (assuming you have updated your game) and they can't do it unless they have discovered you. So you should have discovered them as well.
Nope, I only had 2 minor Tzeentch factions discovered. Since it took 2 settlements in 1 turn... I bet not only the major one can do it. Unless they can do 2 in 1 go?
I fixed it by trading with cathay "join war against", so now I should be at war with all of them. I lost most of my province tho, gurrr! :)
Edit: btw you go to war with them automatically anyway as kislev. So I defo had not discovered them. Weird stuff. bug maybe?
Sounds like it might be bugged then. They supposedly changed it in patch 1.3.

If you are using mods that might off course also cause it.
It is not that I did not believe you, was just saying the facts on my end. Incase you missed my edit: I'm playing Boris, so they auto declare war on all chaos factions on meeting anyway. So I guess that confirms the bug.
No worries, i had to look it up just to make sure i wasn't having delusions and making stuff up :P , figured i could just post the source while i was looking anyway.
While its a long shot i would try verifying files and stuff. Who knows, something might have gone wrong in the patching so that one of your files are the old version or something (or your game might not have updated to the latest patch at all).
Anyway, i hope its just that something went wrong in that one campaign and you won't have to suffer it further.
If that is the case, I hope my save will still work afterwards ;) Thanks for your time.
EDIT: A brand new campaign as well to be clear :)
It is possible to transfer settlements diplomatically, but I am not sure if the AI can initiate that - I have not seen that, and certainly, you have to accept for it to happen.
Was it a double transfer or single?
Well, single transfer isn't a bug, though. Still, if you really don't want it happening, use this:
