Does 100 melee attack equal to 100% success rate to land a hit?
Is there benefit to have over 100++ melee attack

No, it's not linear, doesn't stop at 100, and is influenced by the target's (effective) Melee Defense.
Also no, other than maybe countering Melee Defense, more Melee Attack is better done through Charge Bonus.

In other words, 400+ MA/MD Prologue Kossars still miss and still get hit.

Hits are apparently calculated as 35 + attacker's MA - target's MD % chance to hit. The hit chance also has a minimum of 8% and maximum of 90%.
