If you get notice that your failure - you know turn X.
You just have to go into the Rift at turn X-1.
At Rift you chose Forge of Souls and teleport there.
After end of turn AI teleports there and end it's turn.

It's not bug.
Even if rifts close while you inside, you have turns to finish CHaos realms.
Next turn all rifts will be open.
But player and AI share different turns, so you have time to go Forge.
Say you're at turn 120, rifts open, the AI who has 3 souls goes into one of the realms.
Couple turns pass, you close a bunch of rifts, maybe all of them.
Lets say its turn 125, the AI gets its 4th soul and gets out of the chaos realm.
Now all the rifts will reopen again.
The only way no one could go to the forge of souls is if the blocking building is actually built in every province in the map.
Now if you are somewhere deep in your own territory and have blocked every rift from opening and have no way to get to a rift in time with your main lord's army, you could indeed be screwed.
Reload an earlier save like already suggested and try to get to a rift in time, burn the blocking building in your own province if you have to.
I think you use saves, so you will know - what turn game ends.
So all you have to do - enter Forge before AI and attack it next move.
AI do not build rift protection buildings, so it is technical matter, come to rift at some turn.
Rift spots - are always the same.
AND BONUS: If you are not glitching Slaneesh sword, you will get 4 souls first. AI offen fails at attempt.
9 + player sides to get 4 soul each time Rift open.
The point is, if the AI goes into a realm, they'll get a soul, someone or another.
Don't know about your campaigns but I've seen it happen all the time in my campaigns, the AI goes into a realm and gets defeated, or they decide to leave.
Have you really had every single campaign where 3 AI factions have 3 souls at the 4th cycle of rifts?
Most of my campaigns maybe 1 or 2 AI gets a soul per rift cycle, they more often than not don't manage to get the soul when entering a realm.
In the first campaign, IIRC, there were two factions with 4 souls after the 4th cycle. In the current one there's one with 4 souls after the 4th cycle (currently at the 5th cycle, I'm getting the last soul after ignoring the first cycle), and another one with 3 (because he didn't just go into the realms in the 4th cycle).
To note that, in this current campaign, I'm using a mod so less rifts spawn, instead of basically one rift in every province, so it takes a bit more effort/luck to actually go into the realms (and Kislev AI still managed to get 4 souls in 4 cycles).
But yeah, 10 rift cycles I've witnessed, and if the AI enters the realms then someone will get a soul.
You are wrong.
At normal dif at early turns AI can be beated by mobs army.
I used Slaanesh glitch, killed other leaders in Rift and I was only 4 souls at cycle 5.
You 4 stars enemy only mean, that you barrely get game idea and tactics. Read 1 or 2 guides and you'll see other's game.
And I'm playing very hard by the way.
I also got 4 souls in the 5th cycle because I wasn't in a position to go into a realm in the first cycle, but that doesn't cancel the fact that 2 AIs got 3 souls in 3 cycles, and one of them got the 4th in the 4th cycle.
Fact - in my two (relatively small sample) very hard campaigns I've played, IF an AI went into a realm then they ALWAYS got a soul. There was not one occurrence of the AI going into a chaos realm and not being able to acquire that realm's soul.
