Only if you are married with another player can you have two kids, so that each player can have an heir to continue with when they die.

If you marry an npc, you can only have one child.

I had two children in the Oxbow doing the following: I played with one character until she died of old age. By that time her son ~40 was long married to an npc and had an adult son. When I took over as the heir I could flirt with my wife and ask her to have a child as the game somehow didn't recognize the first child. So I got two children and 20 years later I could choose who I wanted to make my heir.

I don't recommend doing this, as it can mess up the game in the future. It's a bug that'll need fixing.
