Wild West Dynasty in a wild west setting already came out recently, but it's still very early access so you might want to wait a bit til it's more polished and has all the systems working.
Sengoku dynasty comes out this year - similar but in a Japanese setting.
I don't know of any existing ones that have the same concept. Manor Lords might be one that looks promising as well, though I think without the dynasty part. It's also supposed to come out this year.
Those modders have been scrubbing the code, and have already fixed the major bugs (Hotfix !) or its on the roadmap to fix. These guys really love this game and despite the fact they all have full time jobs, have really put their hearts into this project.
Having said that, on to the game....
The first big thing is that this game is voxel based. You actually dig through dirt and mine through stone. You can level land for farm fields. Or carve a fortress out of a mountain top. Wanna dig a moat? Yep (but you'll need the "Water in ME" mod for the H2O)
There are quite a few similarities (in the early game at least) between the two and some huge differences. The first major difference is that there is no storyline to ME. It is best played on a server against live opponents. There are roaming bandits in ME, just like MD, but these guys are toting spiked cudgels or rusty swords right from the get go, so avoid them if you can.
The second major difference is building. The mechanics of building are similar (have the materials on hand, swing a hammer, etc.etc.) but the difference is that the building is freeform. You don't build a workshop, you (depending on how much research you have put into the building "tree") build your fort, (or manor house, or castle....) and then you build a workbench (there are different types for different functions). Your settlement doesn't have to look like every other settlement anymore. As much as I want Toplitz to buy the rights to ME from KSH and merge the two games, I'd be overjoyed if they just changed the building system to ME's.
Third major difference... remember me saying there are no storylines? Aaaand no NPCs other than the Barbs and some deer. There are some great mods that add defenders and scouts and a smith and even ones that give the Barbies flaming swords and a serious bump to hp.
The graphics aren't as good as MD, but ME wasn't built with Unreal either.
I've talked to one of the guys who are working on it (ME) and there are multiple updates already in the works.
If nothing else, the game is pretty cheap, especially if you wait until one of the sales to buy it. I think I paid something like $10.
Go check out their workshop to see some of the crazy ass ♥♥♥♥ you can do... hot air balloons.... steam trains.... trebuchet and ballistas and catapults... there are old west and WWII mods. It's basically a construction sandbox with the possibility of getting shanked by Barbs or your neighbor attacking you because you started a claim war (you can claim map grids but you gotta pay in coin or materials to add time to the claim.).
I'm not really doing the game justice. Just get it. You'll have fun. If you do get it, let me know and we can team up on a server. It really helps to have more than one person in your House.

i /guess/ fallout 4 counts, but i cannot recommend anyone give bethesda money.
i tend to like the classics: simcity, dwarf fortress, rimworld. kenshi is great. actual colony sim mechanics could probably be added to project zomboid and/ or cataclysm: dark days ahead.
