Never have had an issue climbing ladders. Not on my old rig ( 1000+ hours )or my new rig ( new play thru 100 +/- hours ) . Dunno what to tell you.
I for one ran into basically no trouble with the game during my time with it. 2 crashes and two soft locks that got fixed by reloading a save.
Then there's the other end who've had nothing but troubles to this day and then there's everything in between.
Nah i don't think that's it. The issue seems to be the same as with mantling.
When you try to jump up and grab a ledge, you'll quickly notice that actually grabbing onto it it's very dependant on your camera, if you look too far up or down you just won't grab on.
This seems to be in effect for ladders aswell, when you reach the top you can fall down if the camera is looking too far up or down.. hard to explain accurately in text.
So it could be something as simple as the guard rails on yellow ladders make you change how you're moving your view without even thinking about it.
There is one repeatable bug with ladders though, if you jump onto a ladder, not only will V's "grabbing onto the ladder" animations play in a slightly wonky way, but it basically garuentees you'll fall off when you reach the top, regardless of where you're looking.
As for the OP's issue.. uhmmm... i don't even. Not even.
It most definitely is a you problem. Never experienced this before
