I'd add more mid body sliders, mostly separating tights from groin and torso, and add a way for type 1 and type 2 to have their own separate saved sliders just like humanoid and cast have right now. I'd also add voice slots for each save too, so people won't need alt characters and can have all the different genders and options on a single character. Maybe even remove the need for a shape ticket so we can save different shapes. I suppose this won't happen because part of the fashion market goes around shape tickets and having to get two of the same item to bind to the whole account.
As for pvp, the first PSO had it (on the Game Cube version, and if I remember correctly the Dreamcast Ver. 2 had it too), and it was kind of fun, albeit a bit on the unbalanced side. And that would be the main problem with NGS. I'm hardly a pvp expert as I avoid it for the most part, but I believe the current classes would be unbalanced when pitted against each other, as they were created with coop pve in mind.
Can it be done? With a lot of balancing and effort, maybe yes. But I doubt the dev and the main community this game caters to (meaning Japan) has such a thing in mind right now.
People should stop trying to shoehorn in PvP in a PvE game. They tried that in base PSO2 with Battle Arena and it was pretty bad. People only really did it for the weekly Star Gem reward and it was mostly done through win trading.
Your "another version of PvP" is basically the weekly ARKS Record rankings we have now in NGS. Beating certain cocoon (somtimes geometric pyramid) with the highest setting and lowest time or doing the arks field races/parkour with the highest points.

Actually, this feature was in base pso2, they should bring this back. I think this is what gave me the incentive to upgrade my gear. I actually loved this feature. Love seeing my name on top of the scoreboard and comparing other players' numerical values. It boost my Ego
You should ask about this feature on Twitter/X #NGSHeadline if they plan on adding it to ngs or not.
And yet 80% of the player base creates only abominations and slap the "cute" in the description
As for PvP, did I miss something or isn't "PvP" in base game totally independent of a person's equipment? You go into the arena without a single piece of equipment and pick them up off the floor. And die in 2 hits. lol.
also this game doesn't have any one specific style. the character creator is flexible to the point you can specify any style you want, and use accessories to go beyond the base mesh and expressions.

as far as the player character is concerned, anyway. the NPCs do follow a certain style and you may find that generic, but NPCs looking all constipated didn't stop me from enjoying my character which definitely did not look as bad as them, back in PSO2.
speaking of PSO2, facial customization has come a LONG way since it. the variety i've seen people pull off in NGS, on both Global and JP, mogged it so hard it isn't funny, while every nerd still clinging to an old face variant looks the same to me nowadays.
this was actually a thing in base seasonals and something that should be brought back. the rewards were negligible but it pushes you to play better, and with luck, it might even communicate to normal attack spammers that it's NOT what they should be doing in a multiplayer environment because they're doing 1/10000000 what the other players with brains are doing.
