Difficulty Related Achievements

Hero
Complete game on any difficulty

Super hero
Complete game on I AM DEATH INCARNATE! (or ÜBER)

Über hero
Complete game on ÜBER

I recomend to finish this game on any difficulty you like, get all other achievements in the proces and then, BEFORE YOU START NEW GAME, replay last chapter.
Simply select it in main menu and replay it on ÜBER. You can skip a lot of fights and final boss is super easy.

Final Boss Fight

Boss reacts to noise so move every time you shoot. Stay on the left side and crouch all the time.
You can use granades to get monsters attencion to incoming natzis.
Shoot at his mouth and after you kill him you will get Hero, Super hero and Über hero achievements.

Nightmare Levels

You can find locations of nightmare levels in "All Collectibles" section of this guide. To get Die, Grösse, die! (Complete all nightmare levels) you have to complete them all in one playthrough. You can replay chapters to get them but if you start new game you will have to play them all in that game.

Gold Medals

First of all you have to unlock those challenges by simply finishing the game.
To get gold medal you have to play on at least I AM DEATH INCARNATE! difficulty.
Each kill grants you point and you must reach certain threshold to get gold.
Always aim for the head and always try to combo your kills.

Below you will find videos with short description of my strategy.

Research centre combat master
Achieve gold medal for this challenge map

Just kill everyone with headshots. Also try to combo kill as many enemies as possible for more point.

Prison docks combat master
Achieve gold medal for this challenge map

Stealth kill first commander and go loud.
Aim for the head and try to combo kills as much as you can. Move slowly towards second commander and kill him when hi's last man standing. Kill all enemies before you leave, there will be 2 or 3 more waves.

Keep foyer combat master
Achieve gold medal for this challenge map

As before try to headshot everyone and chain kills. Don't rush it, kill everyone and then move forward.

Cable car platform combat master
Achieve gold medal for this challenge map

Same as before - chain kills and headshot everyone. Use sniper rifle to get uber soldat.

Caves combat master
Achieve gold medal for this challenge map

When you get to the elevator don't push the button, another wave of enemies will spawn. Kill them for more point.

Paderborn bridge combat master
Achieve gold medal for this challenge map

Hedshots and combos as always. Stay in cover and clear the area before moveing out.
Use sniper rifle to kill uber soldat and run for his gun to kill robot dog.
Rest is just a mop up.

Wulfburg square combat master
Achieve gold medal for this challenge map

Hide in the tower at the center of the square. Snipe two uber soldats and mop up.

Workshop combat master
Achieve gold medal for this challenge map

This was quite hard for me until I found a great camping spot at the middle of the stairs leading to the exit.

Bathhouse combat master
Achieve gold medal for this challenge map

This is the hardest one. There's not enough enemies to get gold just with headshots so you have to get pretty high combo bonuses. I found a good spot to camp but it turned out that I had to played it on UBER so enemies were more aggressive and that allowed me to get high combos.

Graveyard combat master
Achieve gold medal for this challenge map

To get gold you will have to get to the side area. Other than that there's nothing special about this challenge - headshots and combo kills as always.

Perk related achievements

Any perk can be grinded. If you find a good spot to get a lot of progress towards one or more perks you can reload a checkpoint and do it all again and the progress will accumulate.
For example you can pick up 10 helmets, reload and pick the same helmets again. Do this 10 times and you will unlock Armor I perk.

Eagle Eye

Unlock the Eagle Eye perk

Kill 20 Commanders silently -easy.

Health upgrade I

Unlock the Health upgrade I perk

Overcharge your health to reach 200

Health upgrade II

Unlock the Health upgrade II perk

Overcharge your health to reach 250

Health upgrade III

Unlock the Health upgrade III perk

Kill 5 enemies in a row while overcharged

Three very useful and extremely easy to get perks. You can get all three at the start of the game.

Armor upgrade I

Unlock the Armor upgrade I perk

Collect 100 helmets
Easy to do and you can do it where I;m doing Armor II perk in a video below.

Armor upgrade II

Unlock the Armor upgrade II perk

Overcharge your armor to reach 200

It can be easily done in chapter 4 Wolfenstein Keep. Ther's a hidden room with +75 Armor and a lot of other bonus armor and helmets.

Ammunition upgrade

Unlock the Ammunition upgrade perk

Obtain 100 dual-wield kills

Grenade belt

Unlock the Grenade belt perk

Kill 3 enemies with one grenade
Very easy in chapter 6.

Vampire

Unlock the Vampire perk

Kill 25 enemies using stealth takedowns

Kampfpistole ammo storage upgrade

Unlock the Kampfpistole ammo storage upgrade perk

Obtain 50 Kampfpistole kills

Schockhammer clip upgrade

Unlock the Schockhammer clip upgrade perk

Obtain 50 kills using the Shockhammer

Bombenschuss clip upgrade

Unlock the Bombenschuss clip upgrade perk

Obtain 50 kills using Bombenschuss

Carry heavy machinegun

Unlock the Carry heavy machinegun perk

Obtain 200 kills using the MG46

Reload mash

Unlock the Reload mash perk

Kill 20 enemies with the last bullet of a clip

Empty your pistol and stealth kill enemy.

Quick turn

Unlock the Quick turn perk

Obtain 50 kills from cover

Tough Skin

Unlock the Tough Skin perk

Obtain 10 pipe head explosions

You can do pipe head explosions only in chapter 6 and later.
Those are melee kills with two handed pipe on zombies.

Yes. You have to replay it from main menu.

Just completed the game, with after entering last chapter I changed to Über and restarted whole level, but obtained only Hero achievement. got it after choosing the chapter again in "chapters" and replaying it with the difficulty set already to hardest
You propably need to start the chapter with difficulty, so either when you get to the broken gate where you "climb into 8th chapter you need to switch, or after you get there simply quit and continue playing by choosing the chapter, not continuing in campaign (not sure if this is possible though before completing the said chapter.

TL;DR Anyway changing in chapter and restarting level dont work
