Stock/Brass beast
Sandvich/Shotgun (can be useful in back peddling)
Fist of Steel if you have a teleporter/Gloves of running urgency if you don't.

Brass Beast is best if you have a reliable Engineer with good dispenser placement.
Or Huo Long Heater (That 80 Spy wave in the middle of Nightmare is terrifying).
Sandvich might not be best, since you might get rushed without any heavy damage (puns) to keep the frontline steady, switch to Shotgun to deal with fast moving targets when you run out of ammo.
Fists of Steel is best in my opinion, you could try G.R.U, but the large amount of spam will hurt you a lot more thanks to the mini crits you receive.
Also, instead of having a Scout, you try having a second soldier with more defensive (Battalions Backup) capabilites and with the Beggar's Bazooka, since you'll need the damage from Soldier a lot more then the little buffs you get from Scout.

For Nightmare Mode, you should have 6 Heavies. One should have

Scattergun
Mad Milk
Fan-O-War

One should have:

Rescue Ranger
Wrangler
Jag

The rest should all have:

Beggar's Bazooka
Buff Banner
Disciplinary Action

Things like the Grenade Launcher, Flamethrower, various rifles, Scottish Resistance, etc. are fine, too, but not totally necessary.
