I'll fiddle a bit with it tonight. Try setting a dedicated profile for Fallout 4 in DS4's menu?
I've never set up custom profile, I had alook at it but felt competely lost.
It seems everyone using a DS4 on pc will also be having this issue, so hopefully there is a fix soon enough.

It works like a charm (I have a ps3 gamepad, but it's the same way to do with the ds4)
I'm doing this with DS4Windows, and still having no luck. The issue is with the game.
If you use the controller on its own just by plugging it in, the inputs will be correct in-game.

(PS: let me know if there are any issues, I only tested as far as character creation, but it was working perfectly for me.)
I did that before, everything works fine but the right stick. The camera is way to slow and chaning it in the options doesn't change anything. Also, it doesn't seem to recognize the X and O button in the menu.
Well, at least for me.
X and O were working for me in the character creation menu and main menu - did you mean the later menus?
I cant comment on camera speed as I havent gotten past the character creation, but they seemed to respond fine to the right stick when I moved it. (Turned their heads)
Nevermind. I used a different USB port and now it's working :3
