I highly reccomend this book, there is a little bit on setting up your own custom mod, but its mostly on using unrealed and focusing on mapping, but its a good start.
if you want to get hardcore into the programming side of things, you will want to learn unrealscript, learning c++ and java helps with all of that too.
as for modelling, maya or 3dsmax are great tools for that, milkshape is down to basics but it can export ut2k4 meshes and skeletons and such... the book mentioned above also has a tutorial on importing them in the game... definately worth the money!

3dbuzz has tons of tutorials. Check for the UT2004 Special Edition 2nd Disc for more info
Or you can just use unrealed to decompile all the scripts, ucc is the compiler that should be in the system folder will allow you to recompile them again .. Wotgreal will allow you to add or extract the other stuff thats packed in a *.u file.. Like images and such
