Likely some kind of unintentional mod interaction with Signature Equipment (tho I just crafted it recently so had no time to grind out more damage), but Inugami Ripsaw in Berserker mode is capable of destroying the portals in Old One's Army Event causing the event to be stuck as new enemies cannot be summoned.

Quick Fix is just reentering the world, but might be worth looking into it?
