You mean unique items? Depends on your play style.

I like the Aegis shield. I used to be ambivalent on shields and instead often took a ranged weapon and ammo in addition to lance and sidearm, but after finding Aegis, I carry it all the time and feel nearly immune to random javelins and arrows.

When I did a character focused on ranged, the bow from Egypt was also pretty sweet.

have you asked tavernkeepers. Though I can try to summarizes them all:

In Londinium you can find Caesars sword
In the caves of Dionysus you find a shield called Aegis
In Pyramids is a bow
in valley of kings is a sword.
In Jerusalem temple is a bow (you need to loot the temple to get it)
In Spain there is a "sacred grove" or something like that, there you find a spear from celtic warrior
In Jerusalem, if you are a Christian, you can visit mount Golgotha and find the lance of Longinus
When you are worshiping a god (not Christ or YHWH) you can make "trial combats" (in take an action menu you can pray to trigger it) if you are successful you will get strong ancient weapons and armors
The companion you get from the "Blossom in the desert" quest has a special armor which is very strong
In Hanging Gardens, you get the helm of Alexander if you do the quest

Yeah, there's something there. I don't recall what exactly anymore. Keep looking inside, if memory serves there's a weird twist inside the cave.
