It only happened to a Quarian and at another point I remember using another allies unknown redux mod that added Krogans and I experienced the same bug with a krogan as well. No humans or other modded species ever had the same bug affect them.
Maybe the augmentations mod could work as a possible fix in future cases if such a strange bug happens again. I'd rather not download another whole mod just to fix such a random little bug but maybe it's better then letting a valuable soldier stay irreversibly injured. I might come back and say if it worked eventually. I do plan on getting back into xcom2 for a little bit after all so here's to hoping I get a chance to try and experience this bug again. (Or not, I'd be very happy if it never happened again.)
"When your soldier gets gravely wounded he gets assigned a body part randomly
which needs to be augmented in order to heal again at all.
E.g his legs are critically wounded beyond repair and need to be replaced.
How can I see if a soldier needs an Augmentation?
First, they will be listed as wounded, but don't have a number of days to heal
Second, when viewing them in the armory, the "Augments" button will have a yellow exclamation point.
Clicking it will open the Augments screen with a "Needs augmentation" text."
